GPT-5.6 accidentally deleted user files, OpenAI added 5 layers of protection for Codex

According to CoinMeta, OpenAI, the person in charge of the Tibo product, reviewed the issue of accidental file deletion in Codex and announced the repair measures that have already been put into effect. In rare cases, GPT-5.6 may mistakenly select the wrong path when cleaning temporary files; the most dangerous scenario is when $home is mistakenly identified as a temporary directory, resulting in the deletion of all files within the user's main directory. OpenAI has now implemented five layers of protection: 1. Before deletion, the target is carefully verified; 2. Temporary files are now stored in a dedicated new directory, and system environment variables such as $home are no longer used as temporary directories; 3. High-risk deletion commands are first reviewed, with the system identifying suspicious ones for additional verification, and such commands will not be executed if they fail the review; 4. The full and access settings have been tightened to make it more difficult to accidentally enable them, with clearer risk warnings; 5. Codex has received specialized training to reduce such errors, with additional reinforcement learning tasks added and destructive operations filtered out from the training data. Tibo states that these new measures have significantly reduced such issues without noticeably affecting the normal completion of programming tasks by Codex. He still recommends that ordinary users prefer to use sandbox mode and only enable full and access in trusted environments where recovery is possible.
